Proposals
Required Forms for Proposal Submission
The Office of Research needs various forms for proposal submission. The principal
investigator is responsible for the completion of these forms. The forms and a
brief description of when they are needed are below. Contact the proposal administrator for you Department, College, Institute
with any questions.
OOR Proposal Transmittal Form (PTF)
(required for all proposal submissions and obtaining institutional approvals)
This form must be included as part of the proposal package submitted to the Office of
Research. It remains in the internal proposal file and is not mailed to the funding
agency.
Animal Use Approval Form (Lab Animal Research) [PDF Version] or [>DOC Version]
(required for projects involving vertebrae animals)
The UCF Institutional Animal Care and Use Committee (IACUC) has been established under
mandate from the Public Health Service to ensure the humane treatment and protection of all
animals used in research and teaching. Researchers must file a complete protocol statement
with the IACUC. The protocol must be approved before the researcher can purchase animals or
begin work. Failure to comply with the requirements established by the IACUC is a direct
violation of university policy.

(required for all wildlife animal studies)

(required if proprietary information is going to be exchanged/disclosed prior to project award)

(required for projects involving material transfer)
Invention and Copyright Agreement/Visiting Scientist Agreement (VSA)
(required in individuals not employed by UCF will have access to intellectual property)
Report of Potential Conflict of Interest, Outside Activity/Employment
(Form AA-21 is available through the Office of Academic Affairs)
Request for Processing an NSF FastLane Password
(required to be registered with the FastLane System and to electronically submit proposals to
the National Science Foundation)
Request for Service Fee in Lieu of Facilities and Administrative Costs Rate
(project must meet the criteria established by OOR to be eligible for service fee)
Special Hazards Form
(required for projects involving the use of the following materials: active toxins;
blood, blood products or human tissue; chemical agents; controlled substances; hazardous
chemicals (toxic, corrosive, reactive, explosive, carcinogenic); importation of materials
of animal origin; non-ionizing radiation (laser, microwave, UV); pathogenic microorganisms
(human or animal); radiation producing machines; radioactive materials (isotopes);
recombinant DNA/RNA; regulated bioagents; transgenic or knock-out animals)
UCF Matching Funds for Federal Agencies (MFFA) Form
(Please note that all requests for MFFA Funds should be submitted to OOR on the MMFA request form,
with all required signatures (non-OOR), a minimum of 1 day prior to the receipt of the completed proposal package at OOR.)
